It’s my own format, based on the things that I think are important/interesting to keep track of – I have blank versions of the pages online for anyone else who’s interested.
It’s virtually identical to a scorebook that I have that was provided to me back in the late ’80s. I liked it so much that I made copies so I could have that format forever.
